I was paid by Valued Opinions.  They would have rated an "A" grade  (their surveys pay great, usually about $1.00 per 10 minutes) except for two things:
1) You have to keep track of your "pending earnings".  It can take up to 4 weeks for it to actually show up on your balance.  Plus, I had a couple of pending earnings disappear.  But as soon as I contacted them, I was credited. 
2) You have to have $20 to cash out plus the reward options aren't the best.  Right now, you CANNOT get cash.  You have to choose a gift certificate from Amazon, Blockbuster, Macys, Marriott, Frangrencenet, or Magazines.com.

I've changed the grades for MySurvey.com and Lightspeed Panel from "A's" to "B's".  They lowered how much their points are worth by a little bit and surveys are getting longer and longer while points rewarded are either staying the same or getting smaller.  I very rarely get a survey anymore that meets my minimum requirements, which is $1.00 for every 10 minutes (or 15 minutes, if I'm feeling generous).  I've actually gotten some that only pay $1.28 for 30-35 minutes.  Of course, I don't do those surveys. 

OTX Research is actually a company that makes surveys for clients and uses various surveys companies to reach customers to fill out those surveys.  So you may be asked to participate in a Lightspeed Panel Survey or a Opinion Outpost survey, and they will announce that the survey is hosted by an outside company.  Occasionally this outside company you are sent to will be OTX. 

In my opinion and vast experience of surveys, I advise you to not even bother if the survey is by OTX. 

1) The screening process takes a ridiculous amount of time.  This is the only survey company I know that can have thirty questions just for screening.

2) Often, you will spend twenty minutes on a survey, answer all the questions you would for another company, and then have them announce that you were "screened out."  Now, I have no proof....but from my experience they aren't throwing all those answers away.  You actually did the entire survey.  They just don't want to pay you for it.  Trust me, if you spent 20 minutes answering the normal "is this statement believable, rate 1-10 your opinion of this brand, that brand, the other brand, etc." you've done the survey.

3) The pay, on the rare occasions you qualify, is horrible.  Basic rule for surveys: you want to be paid AT LEAST $1.00 for every 10 minutes of your time.  Spending 30 min. filling out a survey for a $1.00 is stupid.  Trust me, you can make money other ways on the internet. 

4) You only qualify for a survey about 1-2% of the time.  Yes, you read that right.  I've tried over 100 surveys by OTX.  The results: screened out about 90 times.  Five times I spent more than 20 minutes answering various questions about a product, and was then told I didn't qualify.  Three times I've spent more than 20 minutes answering various questions and had the survey "freeze up" on the close to the last page.  When you try to contact them, they say that since you didn't finish the survey, they can't pay you.  Note: I have NOT had a survey freeze up for any site except for them.  Ever.

I requested payment from Opinion Outpost a few days ago and got my check in the mail yesterday!  Fast payment. 

I have to say, that Opinion Outpost is one of my favorite survey sites.  If you haven't joined, I highly, highly suggest doing so.  The surveys are VERY well compensated, payment is fast, and the minimum for cash-out is only $5.00.  Try it out!  You have nothing to lose. 

I have raised the grade for "Valued Opinions" from a "D" to a "C".  Two later surveys that I took part in credited properly without any further action from me. 

I have yet to reach the minimum payout threshold, but when I do, I'll let you know if I actually get paid!

And an update on the survey site "Valued Opinions."
After three weeks of waiting, I did finally receive a response to my multiple support requests.  One of the surveys has been credited.  The other one (the $2 one) has NOT shown up yet, but they said it would.  Due to this, I've changed the grade from "F" to "D".  I will try a couple more surveys from them just to give them a fair shot.  It could have been a temporary problem, and I hate to judge unfairly.  I'll keep you updated!   

Yet another survey site that I have actually been paid from!  Amazing, I know.  Opinion Outpost I rate as an "A".

Cash-out is fast and easy.  You need a minimum of $5 to cash out, but I haven't received a single survey that paid less than $1, so it does not take long at all to reach minimum cash-out. 

Qualifying for surveys is short and sweet.  No answering 20 or 30 questions just to find out you don't qualify for the survey. 

Survey length is appropriate for compensation.  If anything, compensation is a bit better than some of the other sites. 

There are personal profiles you need to fill out when you first join, but they're not overly long.

My only slight problem with the site is that I don't qualify for many of their surveys.  But the longer I'm a member, the more surveys I get that I qualify for.  It is definitely worth your time and effort to join this site!   

 As with the other sites I rate, I HAVE received payment from them before they earn a positive rating.  Overall, I would recommend this to anyone.  It won't take much time, and this is a great survey site. 

Positive-
SurveySavvy pays via check, and there isn't a minimum to cash out (as far as I know).  It says 4-6 weeks for the check to arrive, but mine showed up in about a week, which is very good! 
Initial "portrait surveys" are short and to the point.  No wasting hours filling out info about yourself just to never receive surveys. 
Incentives are great!  And no problem crediting or showing up in account. 

Negative-
Not a lot of surveys sent.  This may because of my demographic or the fact that I am fairly new.

And now back to business.  One of the best survey companies out there is Lightspeed.  Surveys arrive fairly often, screening is usually only a few questions, and payment is fast.  The minimum payout for cash is $10.00. 

The bad part is that they are not currently recruiting members, though they do say that if you see a banner you can join that way.  I am going to put this on the survey page, though, for future reference.  I would check fairly often, because this is a very good site!

With the survey sites, I'm going to grade them instead of just giving a "yes, do" or "no, avoid" due to the fact the survey sites are so extensive. 

The absolute top of survey sites that I've discovered so far is Pinecone Research.  I can't say how much they pay per survey (it's part of the privacy statement), I can say it is the best-paying, fastest-paying, and most interesting site that I have ran across.  Here is the link to their home page: http://www.pineconeresearch.com/index.htm

There is one thing with this site.  You can't just sign up.  You have to find a Pinecone "banner" or advertisement and sign up via the link.  And they're no links/banners on their site. 

However, (hint, hint), if you do a google search for "pinecone research banner", and click on one of the top few results, it will probably lead you to a banner.   After all, we all know that google can find anything. 

So, final rating for Pinecone Research- A+
I recommend to everyone!

Quickrewards, as with any site I share, is free to sign up.  I've earned over $9.00 in the last two months, and haven't had to try a trial offer or anything like that.  If you cash out with paypal, there is NO MINIMUM.  I waited until $9.00, but you can cash out two cents if you want.  You get paid for reading e-mails (max of three a day, your e-mail box will not get clogged up!), clicking on links, viewing offers, and taking surveys.

I don't do most of the surveys, as they are by OTX, which will make you answer 20 pages of questions, and then screen you out.  The other surveys on the site are good, though. And they're promising that the surveys in Feb. will be better.
